The client is advised to approach, either personally, or through mail by post, to provide us with enough information to identify himself/herself to be the rightful owner for the concerned information before the information can be dispatched. If you want to obtain your birth information, please send a request letter with your full name, corresponding address, telephone number, together with a copy of your birth certificate & a copy of your HKID card or passport. (we only record the mother's particulars when the baby is born) and mail to Patient Services Office at E1, Room 5, Queen Elizabeth Hospital, 30 Gascoigne Road, Hong Kong. New articles on hospital events, clinical and health care information are regularly updated. Hope you will visit our web site again.
