I am doing the things that you mentioned about latching on, but still Terence doesn't like skin to skin most to the times ... he's completely OK when feedling with bottle, maybe he's like Gabriel --- born to be bottle baby ... but i am not as "hard heart" and knowledgeable like you do, whenever Terence cries so loud, i "gave up" and gave him bottle ... somehow this also makes me stressful, as i think i won't have any milk sooner or later, so it's a vicious cycle ... what i can do is only pumping ... sigh!

Can you explain more about 猛長期? When will that usually happen and what to expect?

Why not successful? Is he able to latch on? My nipples are short too and the nurse taught me to squeeze my breasts flat enough to be latched on by Amanda. Now, she can latch on quite good. But, she prefers her new bottle to my breasts, especially when she's hungry and my breasts are so engored.^sigh^ If you have already tried your best, it is fine to let Terrence to drink from bottle. It's fine to give him formula if you need to supplement. Honestly speaking, if Terrence is 100% breastmilk baby, you need to supplement him with Vitamins D because breastmilk does not carry enough.

For Amanda, she's not 100% breastmilk baby. We sometimes give her formula but not more 1 feeding per day. My hubby will give her formula when Amanda is eating crazy and still not full. I think that she is now at 猛長期 again. She is more awaken in daytime and she can wake up and eat every 2 hours sometimes. She can have only 2oz at one feeding but have more than 5oz in the next. So, I would rather feed her on demand until she is happy now. But, watch out, don't feed Terrence every time he cries. He may just want to be held, or diaper change, etc.

the reason why Terrence is so fustrated with your nipples, because it is much easier to suck from a bottle.
Nipples need certain stimulation before the milk really comes.. but for teats on the bottle, once he sucks, the milk comes right immediately.
So, that's why he sucks so hard.. and if he doesn't latch on correctly, you feels so bad and sick of it, at the same time, he doesn't get what he needs "milk!!!".. therefore he refuses to suck on it.
Gabriel has that problem.. and that is what we call {nipples/teats confusion}... Gabriel and I really had a hard time to cooperate with each other..

BB會特然改daily routine, like 1st month old, average, infant eats every 2-3 hours with 2-4oz BF.. but in this period, he will eat every 2 hours or less, but he can finish 4oz, sometimes, he will cry for more.
3 months old, infant may be fed every 4 hours with about 4-7oz.. but he will ask for feeding every 2 hour or 2.5 hours for 6oz each feed..
and they do gain a lot of weight.
What I will suggest you... FEED ON DEMAND. that is the way to comfort them.
IF you don't do so, your infant will lose the TRUST from you. that is really no good.

I know that u r a great Mama and very willing to teach others how to take care of BB. My son just born 14 days and I BF him, during free time, I will pump up some milk for him to eat. But in recent 2-3 days, he doesn't like skin to skin. He push me and turns his head away from my breast. But if I give him bottle (pumped milk or formula), he eat at once and eat very fast!

Even he tried to skin to skin, he can stick to my nipple and his month easily lose out! I tried very hard, each time 15 mins for each side and seems the process is very hard for him. What can I do??? Can u help?:-(

I know how you feel right now, as I have gone thru that with Gabriel as well.
When he was born.. what I can say, he is born to be a bottle baby.. he just "not able" to latch on my nipples.
It really takes time to establish breast feeding, and really.. believe in : Practice makes perfect
I would say, because it is easier to get the milk from teat than from the nipple, that's why, your infant will ignore your nipple.
Gabriel has that problem too.
Are you a working mom or a stay home mom?
If you do work after your maternal leave, and you will not think that 100% breast feeding to be your mission. Then, just take it easy, try when your infant is not fussy but do not take it very serious. I always tell all BF mom, Don't get upset if your infant becomes formula fed or half formula fed, as we tried to provide BF, and still,we provide the best for our infants as we would.
If you are a stay home mom, have you breast feed every 2 hours, how many times you skin to skin and you pump, plus you use bottle for feeding either formula and BF?
Give me more information and see what we can do

I am a working mama and will need to return to work on late Nov. As my bb - Alfred eat so much and I don't have enough milk, so I BF + formula almost everytime (only in the mid-night as I'm too tired, so I only give him pumped milk or formula, normally I will have at least 2oz pumped milk and if BB skin to skin not eat too much, I will have 3.5-4oz pump milk for next round). I feed him 6 times per day (actually Alfred control the eating time (feed him on demand), as every time he wake up, we change diapper and then skin to skin for totally 20-30 mins, even he doesn't like to, I tried push my breast close to him month to taste my milk). Then change to pumped milk or formula.

I think he used to silicone nipples, as milk flowout faster, and my nipples not so 凸, so he doesn't like to eat, I'm trying to find out other silicone that similar to human nipples with flow not so slow, just tried Nuby an dNuk Gerber size 1, but both are too slow but seems that they both can make BB open their mouth large and with less gas go into BB's body!

I will try to continue this and try to make my nipples 凸, if he really doesn't like skin to skin, then I will pump up my milk for him to eat as I believe that BF is much healthy.

Nellie_mami, could you please tell me which brand of orthodontic nipples u r using so that I can try! Thanks!

1. you need to buy the nipple shield, which will correct theshape of your nipple which will help Alfred to latch on easier.
2. Good on you that you will let Alfred latch on for a while before you give him the bottle, 10-20 min will be enough, as if the position is not correct, or Alfred can't even able to latch on correctly, he will be suffered from starve. that will stress him out.
3. as you will return to work, then you need to stock up the milk from now.
4. about the teats. Gabriel did use NUK, did you make sure that the "air" port is open. as after you sterilsed the teats and bottles, the "air" port will seal together, you need touse your finger to pull and open it everytime before use.
The milk should flow much faster.
Other brand like Avent, the first year..etc that you can see in babiesrus, they all design for breast feeding baby on bottle --feed with express beast milk, I can't tell which will be good for Alfred, you need to buy them and have a test drive. every baby is unique.
5. Good that he is a bottle baby, so the care giver will be no problem to take care of him after you return to work.
Plus, he will wean off night feed when he is ready
6. you need to feed Alfred at least 8 feeds a day, no matter he doesn't need to, you need to wake him up and feed him every 3 hours. If you tell your pedi that Alfred only takes 6 feeds a day, she may say that you put Alfred in danger. By American Pediatrician Asso, infant within 1 month, need to be feed at least 8 or equal to 8 times a day.
With at least 6 wet diapers and at least 1-3 bowel movement.(100% breast feed infant should at least have 3 bowel movement).
The reason to feed frequently, because they burn calories very fast.
The reason Alfred needs to suck so fast, because he suffered from hunger. But after overfed. he then keeps sleeping.
You need to adjust that.

If you use the nipple shield and your son can suck it correctly, you can take it off after a few minutes and let him latch on directly to your nipple. Usually, after a few minutes correct sucking, your nipple would be pulled longer and should be more easily to be latched on. My nipples are also flat, the post-partum nurse taught me to squeeze my breast flat with my fingers and push my daughter's head towards it once she opened her mouth. At the beginning, it was so hard to find the right way and feeding was a nightmare to me and my daughter. But, she picked up quite fast and I tried different positions and angles. Now, she can latch on with no problem plus I have established my milk flow to 4-5oz for each pumping. Skin to skin is really important for milkflow establishment. If it is not really possible, keep pumping every 3 hours. Eat the food which can help increasing milk flow would help too. I ate a lot of pig's feet, fish, papaya and milk.

I'll change Alfred's feeding schedule to every 3 hours and just asked my husband to buy nipple shield and will try to use tonight! hope that it can help la...

But before that, may u teach me how to use the nipple shield? I've ask my husband to buy the Medela extra small one for me from Babies r us. As I still not yet get the sheild, am I just put the sheild on my nipple during the time I feed my BB? Or do I need to wear it all the time?

Better to get a standard or small one (look for the 24mm one). The XS one is designwise, not easy to stick on your breast, plus it is still not too long for Alfred to latch on. You only need to use it when you feed Alfred. Read the instruction and you can know how to make it stick on the breast - Dip it into warm water, upfold the side and put the center part all over the nipple and fold down the side. Since the side has been dipped into water, it will stick on the breast. After using it, wash it with a little bit of handsoap or liquid soap, rinse it clean with warm water and store it in a dry container. Next time when you use it again, rinse it with warm water and follow the above. If you keep it clean and dry, it can be used for a long time.

Thanks! I start using the nipple shield la... which is 16mm, it's good and Alfred tried to push me 1 or 2 time them he stick on the nipple shield and his mouth keep sticking on it for 15 mins without stop. It makes me very happy, and I've used 3 time, but still can't feed Alfred without it. I will try everytime and will try whether he can feed without it or not! Thanks a lot!
As I change slow flow bottle and skin-to-skin, Alfred now seems eat less than before and he wake up every 3 hours, I'll try to eat more food which can hlep to make more milk.

These days I have been breastfeeding Terence before giving him formula ... latching on has improved after following your suggestion ... however since yesterday he cries even when he's sucking and also uses his hand to push away my breast ... he's very forceful in terms of a one-month old ... does that mean he doesn't want bf? what should i do?

Concerning pumping, the quantity hasn't improved even i have skin-to-skin more with Terence ... sometimes i pumped for 20 minutes for both breasts and only ended up getting 1.5 oz, so frustrating!!!

Also, now i am using Medela Swing and it comes with the standard size breast shield ... i feel the strength is not that strong ... should i change to a smaller breast shield??

there should not be the reason of less suction power because of the size of the cup As long as the cup is cover all over your breast. normally, size 24mm is will be the right one for most of the mother. When you pay attention on the cup, you will see that there are letters on the cup.. tell you the size of the cup. that size is according to your size of nipple and arerola, not the size of your breast.
Did you increase the suction power to maximum?
Did you increase your frequrency of pumping to every 2 hours, pump 10 min on each breast or just wait until the finsihed of 1st letdown?
Did you drink enough soup, water, juice and milk each day? Did you eat well every day?
Did your family leave and return back to Hong Kong?

Did you eat well every day? i drink soy milk and water; after my mom and grandma left, i really didn't eat that well and that much ... probably this is my main problem ... i don't have time to cook ... always need to take care of Terence, as he doesn't sleep much during daytime ... also, not enough rest, too ... sigh!!!

as you can not able to maintain to empty your breast every 2 hours, therefore, the breast feeding is not being established, therefore, you find it difficult to express your milk even you switch the suction to maximum level.
As all of us suggested.. the frequency is very important.
So do the diet.. at week end, you need to go and buy the papaya (green one) and fish back home, cook some papaya soup!
Are there any chinese super market close to your house? do you drive? or do you call car service to do groceries shopping?
If you really care about breast feeding, you have to skin to skin Terrence every 3 hours, 10-15 mins, just 1 side, then supplement with formula..
If you miss your engorement, that will be very hard for you to achieve the goal --> continue BF
Let's see if the 21mm works well.
I am using 24mm.. (4 of them) as I need to bring 2 to work.. When it is the begining, even the minimum suction hurts me a lot. it takes me almost 1.5 months.. to have enough coverage, to increase the suction to maximum..

我生完zoey後, 都曾經都pump唔到奶呀.
我想問吓你pump完後, 有無按個胸時發現一舊舊硬物係胸入面. 如是, 咁你可能乳腺未通呀. 咁我見意你下次當你發現有硬塊時, 就用d力按吓佢/打圓咁按摩吓佢, 要按到痛喎, 咁你按一陣再pump咁你就會有多d奶出. 有時因硬塊太大我按到眼水都標出黎. 但你真係要忍得痛先得.

另外, 我返工後, 我媽咪己好少整魚湯我飲, 所以我只係飲豆漿, milk同升3,000 ml up 的水, 而我每次pump奶前都會飲一杯奶同2杯水, 先pump奶. 之後, pump完15mins後, 我再飲水再pump, 但可能未有奶出, 但繼續pump, 一陣就會let down.

我前2日半夜pump到8OZ, 不過, 要pump左1粒鐘呀.

另外, 你可以買orginic mother milk的tea bag 飲, 我飲後, 好似上奶快左.

Frequency is very important for pumping. How many times you pump and skin to skin in a day? If you want to increase milk flow, you must skin to skin Terrence more. Pump when he's not able to empty your breast or miss a feeding. Now, I pump 2-3 times and nurse Amanda 5-7 a day. From my experience of 2 pregnancy, skin to skin is a very important stimulation. In my last pregnancy, I believed that I can get more milk if I wait longer between each pumping. It's true but not really effective - I could get 3 oz if I puped every 3 hours but I could only get 1 oz more if I pumped every 4-5 hours.

Obviously, your diet affect your milk production. If you don't eat enough, you cannot produce enough milk for Terrence. He eats what you eat so the choices of food are also important. I usually drink a glass of water before pumping and eat when I feel hungry - I cannot predict when Amanda wakes up so I try not to let myself starved when I feed her. Sometimes, I eat when I feed her at the same time. Since your family members have gone back to HK, stock up your refrigerator with food, keep some milk and bread which do not require cooking. You can also cook a big pot of rice and keep the rice cooker on or use the microwave to heat it up. I usually make more food at dinner and have some leftover in the refrigerator. When I need to eat, I simply put it into microwave (I know microwave food is not too healthy but better than starving my girl).

Help again! Alfred likes the nipple shield the first 2 days and now, he seems just like play around it and doesn't like to eat it. When I remove it and put his mouth to my nipple, he just can stick on it for 5-10 seconds, then open up his mouth and cry, or when he is eating, he has lots of sounds and mouth lost from my nipple soon. I've checked my milk flow, but seems it's enough, as after he mouth off, I still can see milk drops from nipple (without adding any presure). I now can only feed him on my right breast, as he hurt my left breast so much ( it's now very pain, and I will wait 1 or 2 days for recover then will try to BF him with my left breast again, now I just pump milk from my left breast for him to eat).

Normally when Alfred wakes up every time, we help him to change diapper, then will skin to skin for 20-30 mins (both breast) then give him 1.5-3oz formula (depends on how much he drink). Normally he will finish 2.5 oz and then goes to sleep and hardly wake him up. But he will wake up again in 2.5 hours............... does it show that he is not enough? Or skin to skin to long to make him so tired? How come he can't sleep 3 hours or longer? or does any wrong with my milk to make him become hungry so soon?:-( I really want to keep BF him..... so please tell me if it is normal.... Please help!!
