Sorry... switch 你個message 過嚟呢邊.
Base on your objective data, Isaac 係超出正常比例.. but breast feeding baby is fine.. Don't worry too much.
The nurse and peds in hk still have the same old chinese mind.. They always forget one thing.. Breast feeding baby is fed on demand, and also, breast + Bottle mixed baby is always heavier than others who only base on 100% breast or 100% formular.
The diet for your son is OK.. I don't see any error in there.
Keep going, you are doing a good job.
When Amanda was 4 months old, she was 18 lbs as well.
For Gabriel, I didn't top up any formula, so he is always within 25%-50%.
Isaac 只係喺厭奶期... 喺厭奶期時只要offer feed every 3-4 hours, until 佢嘅胃口 comes back, 唔好focus Isaac 食嘢.. 只要maintain 住, 冇dehydration就ok架喇...
你話我衰都好喇.. BB 細時肥小小, 一入學, 成日病就自然瘦.. 放心喇

我最近遇到一個問題, 姑娘話Isaac overweight. His information is as follow :
Age : 4 months
Weight : >120 percentile (9 kg)
Height : >90 percentile
Head circumference : > 90 percentile

約了今個星期六見健康院醫生, 但我覺得Isaac食奶不多, His schedule is as follow :

06:00 - Breast feed (one side for 15 mins.), then he fell into sleep.
07:00 - Breast feed ( another side for ~15 mins.)
12:30 - 100mL (pumped milk from stock)
15:00 - ~100mL (pumped milk from stock)
17:15 - Formula 120mL
20:00 - BF both side (around 15 mins. each side), then sleep through the nite).

But on last Sat. it is so weird. I only can bf him 5 mins. each time and took 40mL formula during the whole day. He refused to eat, but didn't look like hungry and never cry for milk. Should I feed him on demand or we (my maid and myself) should feed him every 3-4 hours regardless his will?
My maid gives him formula or mother milk whenever he can't sleep well ...or has hard time falling into sleep. I feel that it is not right...but Isaac does seem sleep better....could you please give me some adivces on this? Thanks again....you are so sweet.

Dear Nillie, Yeah, thank you so much ar.......我想問下我land 了San Fran. 之後, 怎樣找醫院和兒科醫生monitor住Isaac's growth 和打針呀? Insurance 方面又要怎做 so that we will get full coverage? 我打緊子宮頸癌嘅vaccine, 但最後一針要係USA 打, 我應該係H.K.買定支vaccine, 係USA找醫生式或自己打? 定係係USA才買? 曾不曾有Insurance coverage?

美國係冇母嬰健康院, so, 你要自己揾peds 同 family doctor (for you-- you need a OB/GYN, a PCP{primary care physician}), for BB needs a Peds, your husband needs a PCP.
我唔清楚S.F. health care systen 係點, as 個個州都係unique.
最好請教住S.F.嘅媽咪!
你要帶你哋所有針咭嚟US, that is a must!
你未有工做,但要health insurance full coverage... 每個月嘅health insurance(vision, dental +medical) 1family有可能過千元美金架.. (for my family-- hospital paid full coverage for us as I am a RN-- that is USD$1400 /month if I need to pay by myself)
so 你未有工做之前, 開頭可能要pay cash 先呢..
我唔suggest 你買針嚟US揾MD打, MD未必會為你take risk.. 因為隻drug唔係佢prescribe 俾你.. 出乜嘢事 even you promise on your own risk, 醫生都好少咁勇呢 Also, even 你幫自己打.. 係咪 IM先? IM嘅話... 你點幫自己打?
你最好search 吓 US 有冇嗰隻藥... 等US 嘅MD開就會好啲.

你到咗三藩市我可以介紹我個OB／GYN俾你，佢同佢個partner都好出名，我好多朋友都係佢地接生同做婦科檢查。Pedi 我可以refer 我兩個BB的醫生。就算你先生未有工做前，你地都要有medical insurance, 因為BB要定期檢查同打針，你可以用政府醫院，不過就又遠又要等好耐，我試過同我媽睇醫生等咗成5－6個鐘，而且好難有固定醫生。記得針咭，就算入托兒所都要有針咭做記錄。

花旗太太生活在花旗國，留意時差，你問我未必即時答到。

Amanda 食樽都係唔掂，佢真係好硬頸，寧願喊同瞓，等我放工番屋企先食番夠本。我今個星期開始番工，8個鐘泵兩次，每次都有5－7安奶，唔算多但因為囡囡唔肯食，呢幾日已經有成7、8包奶未食。而家囡囡會食一至兩次夜奶，有時我可以朝早5－6點餵一餐，跟住起身凖備同Spencer 返daycare，7：40搭巴士返工，4點老工接我放工，跟住接囝囝，大約4：45到屋企，囡囡已經等住食，有時我會同兩個B沖完涼先餵囡囡，囡囡可以一晚食2－3餐先瞓。我媽咪湊咗兩日，每日都係只餵到8安奶，星期三到今日老公放假，佢湊仲死，今日只係食咗唔够3安，Amanda居然只係喊同瞓，等我返屋企就笑晒然後勁食。我都唔知點算，老公話我唔應該skin to skin, 特别係夜晚，我其實都想老公舒服D，除非佢肯每晚起身餵囡囡食樽。老公想我夜晚起身餵樽，我話佢知咁囡囡會好confuse, 點解媽咪係度都唔佢食人奶，我驚咁仲難教。而且，我咁餵完樽仲要泵，仲要最遲6：30起身凖備同囝囝返托兒所，我最多都係得5個零鐘睡眠時間，我唔知咁樣頂得幾耐，唯有希望囡囡快D肯食樽啦。

開頭Gabriel 都係咁.. Amanda佢想你返屋企skin to skin 佢嘛..
when Gabriel 3 months old, 我同Gabriel 都struggle 過.5 months, until 佢5.5 months old, 餓扁咗同學識食Avent, 我至放心返返工.. 我好冇用.. 當時日日淨係識喴.. 好在英姨姨夠企硬.. Gabriel 摟佢唔過 (as bed time, Gabriel stayed with 英姨姨), finally, 佢屈服, take bottle.
day time 我skin to skin (as我看夜嘅)..
因為我同你嘅time table 係opposite.. 你有冇諗過老公咁講.. 俾Amanda only on Bottle (monday to Friday)..
weekend skin to skin..? or even weekend on Bottle (but then 好快收奶)
night time, 老公俾bottle, 你去泵奶.. that is the way we are doing... Amanda 係你同老公嘅BB, 大家日頭都要返工.. so, at night if 老公要求Amanda on bottle, 佢就付責起身餵呢.
also, at the beginning, 你餵Amanda, 佢唔食架... 因為真係好confuse 呀!
女仔始終叻過男仔, 俾2-3個月Amanda去adapt 喇!
如果pump 2次, 每次有5-7oz (係each breast or both breast?)-- 記得食prenatal vitamin, DHA supplement, at least 12 beverages of fluid everyday.
frozen 啲Breast milk (in office), then 返到屋企continue frozen, 因為你雪櫃有存貨... when Amanda over 6 months, supplement with solid food (rice cereal), apple sauce, pear puree, peach puree. then, by 7 months, 俾 sweet potatoes, potates mash 亞Amanda 食.. after 加咗solid food, Amanda 會口渴, 就會飲多啲奶.

if service is available, 唔好介意去申請用住先, as BB 要有insurance coverage.. 入一入ER.. if 冇 insurance 要pay cash, 未做檢查巳USD$350, 見過MD 又USD$330.. 唔係講笑架.
我初初到new york, every time check up, or 有唔舒服睇MD, 要at least USD$60-100.. 一枝ventolin puff 要USD$28 (03年價錢)... 真係好貴呀! 我哋冇用medicaid咪洗多好多錢...

Thanks, Nillie, Mami. Wow, US$1,400 per month, that is a lot. My hubby asked if he can wait until he gets a job. His employer will pay for that, rite? SY_Mom sent me some information about insurance. I still haven't had time to look at it detaily.....but I saw some family packages is only US$25 ..... not sure if it is a good plan though.

so many things need to be taken care in this period :(

I got bronchitis and need to take antibiotics, I can't bf Andrea during my course, is it ok to use the pumped breast milk to bath Andrea? will it be harmful to the skin?

Yes, after you land in SF and settle down, give me a call and see what you need. Kaiser is okay and has improved its services. I used to have insurance under Kaiser but now I have transferred to CPMC which has the best OB services in SF. Kaiser is good in a way that you don't need to find your own doctor. The hospital would assign doctors to you and since it's a hospital with all the services, it's more easy for specialist referral. However, it's always a long waiting hour even though you make the appointment in advance. CPMC's facilities are more advanced and it won't take you to wait for a long time for hospital's visit.

In SF, we do have low cost medical plan for low income but not for new immigrants. You may need to buy your own insurance before your husband find a job because Issac needs to have vaccine and regular checkup.

have you checked with Breast feeding association L.L.L. in hk??

short-term use of this antibiotic while breast-feeding, has been guardedly approved.

Here's a list of various drugs considered safe to use while breast-feeding:

Trimethoprim/Sulfamethoxazole

When you are sick, Andrea will get sick as well, if you take any of those antibiotics above, I will suggest you keep breast feeding Andrea, that will help her get stronger and immuned.
