You may also get a statement from Social Security office to state that NameA aka NameB.

I have a paper from SS office (with their stamp on it) stated my 2 names. e.g. Siu-Ming Chan aka Peter Chan (other name used).

Regarding the change of last name, I did not and will not change my last name. 坐不改姓 :) Besides I dislike my husband's last name. The paperwork is one thing, the major thing is I do not like this idea. Many professional female in the US do not change their maiden name as well. You can always use some other security password for bank etc rather than using your mother's maiden name.

I read long article about people changing their last name DIY (not thru external legal service) for their kid (reason is because the Chinese name doesn't sound good or hard to pronounce in English etc). You may try to DIY the name change if you want to. I will see if I can find the link on the name-change story that I read before.

I think if you are not in a hurry to change your last name, it may be easier for you to change your name when you apply for citizenship (and use it in your US passport) etc. It's more mah farn to have more names for the background check etc.
