That is a change of address sheet, but you are adding people to the job as well, which is different. Better attach a letter informing Immigration how many people there are and their ages, have maid sign it.
Sharing a room is ok. My friend's maid has to share a room with 2 other women, 1 is paralysed. As long as same sex, ok la.
