you can post the warning letter to your home address in your maid's name (registered post). As she must sign for receiptance, it can be a legally proof. even she can still said she didn't read it.
From my experience, you do not need to be so serious about warning letter, even if you have given several warning letters, when you fire her, you still have to pay one month notice in lieu. The warning letter does not mean anything unless the maid did some serious thing such as letting your child falling down and need to go to hospital for stiches etc. Otherwise, warning letter because of her performance is unsatisfactory is meaningless to the labour department, you still have to pay her when you fire her, remember, the labour department does not protect employer.

Last night, I discover that my maid do my son's homework again. I warned her not to do this one week before and lastly on monday. But she still disobey my order & do it again on Tuesday. When I discover this, I ask her but she deny. She insist she didn't do that. I was very angry and want to fire her.
