Warning Letter - urgent

Dear friends,

Does anyone have a copy of warning letter to Indo DH?

This afternoon my DH said that she resign and gave me 1 month notice (orally), then she said that she must go out for 3 hours. She's been out since 1:00pm did not come back to cook without any calls and she's still not back yet (it is now 10:26pm) !
