Yesterday, I felt painful on a small portion of my left breast (breast not hard). And a small white spot appears on the nipple today, but felt less painful on that part of breast. Is certain milk duct blocked?
I already massage that painful area, take hot water shower, let baby suck & use milk pump, but still can't help. What should I do then?

milk duct blocked. try using a sterilzed needle (wiped with alcohol) to unclog those white spots la. little amount of milk will come out. it helps but a little painful.
A clear white blister is usually a sign that the bay is not latched on well.
A white spot on the nipple may be caused by a plugged milk duct, thrush, or skin blocking the milk duct and may occur after a crack to the nipple.
If the blister does not hurt, you do not need to do anything.
If it is painful, you can use warm compresses or soak the nipple in the warm water. Another solution is to use a cotton ball soaked in olive oil and place it in you bra to soften the skin and then attempt to peel away the thickened layer of skin. Then try to express from the nipple any remaining milk, sometimes, it is possible to express a thickened string of milk, which will help open the duct and keep it open.
If these treatments does not bring quick relief, you should go to some lactation consultants or doctors to open the blister, they know how to prevent infection on the opening area.
If you try to open it yourself, please make sure the needle is fully sterised, otherwise the would may be infected. After that, you may use mild soap once a day to wash the nipple(not anti-bacterial soap as antibacterial soaps are often strongly scented which may cause the baby to reject the breast).
