For Answer 1 & 2, I won't consider about statement account unless no better choice. You can open a bank account at Bank of China. A passbook and an ATM card will be provided to your maid. No need to deposit any money when opening this account. Only restriction is:
1. If withdrawing money through counter staff, there will be service charge. Deposit money through counter no problem.
2. Bank of China will only accept applications with HKID original formal copy. So if your maid has only the temporary HKID, wait until the formal one being issued before going to the bank to open. Don't forget to bring the green contract and her passport as well.
