2. letter from the employer saying that both of you agree to extend the contract until whatever date, but will not renew the contract for 2 years because of whatever reasons.
3. copy of employer's ID
4. copies of DH's ID / passport pages (with photo and with the entry permit)
5. original contracts (employer/DH). Use the blank area at the end to say that " I, Chan Tai Man, agree to extend the employment contract until e.g., 10February2012" and then signed by employer and DH. Do it on both contract copies.
6. HKD160.- VISA fee
Note: You can apply to extend the VISA for one month from the expiry of the DH's VISA, not from the end of the contract date. In my case, I got nearly 2 months' extension from the contract end date.
Just got the approval this week. No need to use agent. Hope I have not missed anything.
