you may ask your wife to do pregnancy test at the Family Planning because it is much cheaper and quicker. if it is positive, they will issue a medical certificate. or your wife may go to any general doctors. then you may call the hospital and ask them to fax the forms. after filling in the forms, fax them back together with your wife's ID copy and the medical certificate. the nurse will contact your wife to make appointment upon receiving them. hope this may help.
