This is to certify that my domestic helper Miss ABC (Passport Number 123456), who has been working under D.H. Contract J001002, will take the annual leave back to the Philippines from 18 December 2012 to 1 January 2013 (both dates inclusive).
Per our agreement on annual leave, it is hereby assured that Miss ABC will continue to work for me under the same D.H. Contract upon returning to Hong Kong.
