Q(2) contract green form係邊處有得攞?
Ans. From Immigration Dept. I got mine from Wan Chai Immigration Tower's Immigration Dept Enquiry Counter where you can get all forms relating to immigration matters.

Q(3) photo of worker要咩底色, 尺寸如何?
Passport size, 1.5" x 2" will do. My yanyan used an old photo, but of course not too old. Can be the mauve / reddish background used for yanyan's passport (take a look at her passport), can be blue, really doesn't matter, not stated anywhere. The background colour is important only for photos for passport renewal.

Q(4) letter of garantee (appendix A)係邊處有sample?
Ans. Please refer to page 2 of fax. Can just use that, or type it out.

Q(5) 仲有咩文件要預備俾印尼領事館?
Ans. Same documents as that required 2y ago. (Pls refer to P1 of this topic).

Q(6) 印印在2008年2月2日完第二個約, 剛剛農曆新年好忙唔想立刻放佢大假, 上次完第一個約時, AGENT幚我安排佢開始左第二個約後在第三個月(即4月份)才放大假返印尼, 如果自行續約又得唔得甘安排呢? 印印的hk working visa在02/02/2008到期, 她的印尼護照有效至20/10/2009.

Among the forms you get from Immigration Dept (there is a whole pile of forms to fill in, not just the green contract), there is one in which you can tick and choose whether you will allow yanyan to take home leave immediately upon contract end, or within 1 year of end of "old" contract. Need to give a reason (reason I gave : Yanyan would like to take home leave later.)

But MUST check yanyan's passport expiry date, cos the DH Visa will end on same day as contract end date or passport expiry date, whichever is earlier. If passport expires before contract end, then yanyan needs to renew Indo passport first, then you as employer need to fill in an entire set of forms for Extension of DH Visa again.
(Renew passport first, then go to Imm Dept to apply for Extension of DH Visa.)

同意書(見証正本),
印印爸爸的印尼身份証(見証副本)
**見証本=要付$找村內合資格人士見証**

(1)但"戶籍表"好殘舊, 有的破爛又過了膠, 印尼領事館受唔受呢
(2)上次你給我的香港印尼領事館自行續約資料中, 第3頁全印尼文是作什麼用途呢? 我怕自己做留了文件.....
(3)現在距離約滿尚有大約60天, 若現在就開始辦以下文件. 印尼領事館會否因文件太早不接受呢?

印印寄掛號信給印尼agent通知自行續約不經香港agent
驗身報告及出醫生信

到政務處宣誓<letter of guarantee>
(4)可否提供僱主application letter樣本(containing reasons for renew contract without going through an agency)

- Not sure, depends on Indo Consulate. But no harm in trying ga. I think the Indo Consulate is more helpful now than before.

- I got your pm about this, but forgot to bring the page back to ask my yanyan. Will let you know later.

(3)現在距離約滿尚有大約60天, 若現在就開始辦以下文件. 印尼領事館會否因文件太早不接受呢?
- Usually start processing 1 month before contract expire, now should be tooooo early.
In my case, I asked yanyan to go to Indo Consulate ~24 Oct, and got the documents back on 30 Oct. So really no need to do it tooo early.

(4)可否提供僱主application letter樣本(containing reasons for renew contract without going through an agency)

Write a letter. Let me copy that section of my letter here..... wait wait.

1.
For the past four years, it has been permissible by the Indonesian Consulate General for contract renewal cases to be submitted directly by the domestic helper and the employer without going through an employment agency.

2.
Miss (maid) will be able to save a significant sum of money.
In fact it would amount to over HK$2,000 for her.
This is equivalent to over half-a-month’s salary to her, and she will now be able to save the money and send it back to her family in Indonesia.

3.
Her performance for the past 2 years has been good, and we did not require any action by an employment agency for any dispute during this period.
We have enough trust of each other and have both complied fully with the DH contract, so that the need for an intermediary employment agent to protect the interest of both parties is not necessary.

4.
We did not renew our existing employment contract through an employment agency for our last XX contracts, and have not found the services of an employment agency necessary in our case of contract renewal.

Yes. Actually you need to do these 3 things before you go to Indo Consulate ==> can do them now.

Total spent :
1. Declaration re Guarantee Letter -- free (~$500 if you go to legal firm and get it notorized by Notary Public)
2. Doc's "fit for work" letter -- $400 (cos she went to my doc who is O&G; other family doctors may charge less)
3. Lab test (ie medical check lab test) -- $480
4. Indo Consulate admin fee -- $320 + Indo bank handling charge $10 = $330
5. HK Immigration - DH Visa $160

我又有事請教.....
(1) 你個lab test好平呀, 因為我問過有d要hk$500-640. 其實係要test咩呢? x-ray照肺係咪一定要有呢, 有d套餐驗埋hiv愛滋架

(2) 我見印尼領事館要求出示original contract copy, 但我只有一份original, 印尼領事館會唔會收返架? 因為香港入境事務處都要求出示original contract copy才可續working visa

我真多問題, 唔好意思, 麻煩晒你....
