1. Any person will do, but the one for your maid preferably somebody other than your family member (to prevent chance of maid saying she doesn't know what she signed). No need for company chop. Just a signature (on each of the 4 copies) will do. e.g. can ask your friend / colleague at work / 看更, maid of next door neightbour.
2. I don't have actual experience on this. Perhaps those BK mummies who have successfully completed the process recently can help.
(My personal opinion is that the registered mail chit should do - but then there is nothing to prove the contents of the letter being sent. I know one form of practice is to ask the recipient (ie the agency) to stamp a "received" chop with date, on a copy of the letter itself when the letter is being delivered, using local courier service.)
3. The existing, valid insurance policy (for "old" contract).
I have prepared everything except the medical report. I have phoned to 日康醫療有限公司, they said there will not be any doctor report because they are laboratory only. Do I need a letter written by a doctor stating that my maid is fit for work or just the lab test report is OK? The exsiting contract will be expired on 18 Dec 2004. Do I still have enough time? The agent phoned my maid yesterday saying that the consulate will have no time to process my application and it is better to let them to do it. They charge me $2000 and my maid $2000. Is it true or not?

The Indo Consulate says a doctor's letter certifying that maid is fit for work. They won't accept lab test report. Just go to any doctor (maybe your family doctor) with the lab test report and ask him to write the fit for work letter.

If you have time, call up the Indo Consulate tomorrow and say you want an appointment, see what they say. If they still insist on making appointments only 1 month before contract (DH visa) expires, then call them up on 18 Nov to make the appointment.
As I said, I completed the renewal process within 1 week. If you look through the pages in this thread, you will see that there are many ways to go around the timing issue.
