Ans. Yes and No. You need to go once, 親自in person, to the Indo Consulate for the interview with Consulate staff.
Then after some time (normally 1 to 2 weeks later) you can collect all the documents (new contract stamped, Indo maid's passport) from Indo Consulate and go to HK Imm Dept ON THE SAME DAY to complete DH working visa processing. ----- Your maid can pick up the things from Indo Consulate and then go to the HK Imm Dept BY HERSELF, but the point is, you need to make sure that EVERYTHING is ready (including all necessary originals and photocopies and photos, the money for paying the "tax") and duly completed and you need to give your Indo maid items like your tax assessment notice, the HK Imm Dept form which has the dates of birth of ALL your family members etc. private info. And if the Imm Officer asks for some missing info / document / needs something to be photocopied, your Indo maid may not be able to cope and then she has to go again...........
So it is best to go to HK Imm Dept yourself, together with your Indo maid.
= you need to "go" twice, once to Indo Conosulate (for interview; compulsory), once to HK Imm Dept (for sumbitting the appl form for Indo DH visa + payment of "DH tax"; not compulsory but preferable).

Ans. - Should depend on whether all the crucial items are covered, e.g. urine / poo-poo test, chest x-ray; so that the doc reading the lab test report is willing to issue a doc's letter certifying that DH is "fit for work". You had better ask your doc to see what he needs before he is willing to issue the letter; also you can ask the lab to cover all the "crucial" items ONLY.

Thank you bluestar. But I still have a few questions after read all information posted in Indo Consulate, could any BK can help!! Urgent! contract will be expired early April this year.
1) "Health statement letter from doctor attached with medical check-up report" Does it mean I need to bring my maid to do lab test, then ask a doctor to write a doctor report. Here are 2 actions, how much (estimated) of each action.
2) Legalization fee ($480) which is paid to Indo Consulate? or pay to who?
3) How much about the notary fee? (estimated)
4) How much I need to pay to HK Immigration? ($135 + $9,600 that's all??)
5) Does it mean I need to get ready all documents (item 1-13) then make appointment with consulate OR I can make arrangement with consulate first & try to get all the documents before interview?

2. This is more an "admin." fee charged by Indo Consulate. You need to pay $480, whatever they call it. (The Indo Consulate has to put a Consulate stamp/chop on the 4 sets of contracts and interview you and the maid.)
3. Depends on the lawyer.
4. For HK Imm Dept : $135 for Extension of DH Working Visa. + "DH tax". For the DH tax, you can choose to pay in 1 lumpsum = $9600, or pay in 4 instalments over 2 yrs = $2400 each time, for 4 times, at half-yearly intervals. If you choose to pay by 4 instalments, you need to pay $2400 on the day you apply for and collect the new DH working visa at the HK Imm Dept.
5. Suggest you start to get all documents ready NOW. Some items, e.g. the letter from your maid's parents/husband certified by the "village official" needs time for posting to and fro.
But you can only call up Indo Consulate and get an interview appointment exactly 1 month before current contract expires. By the time you go to the Consulate for the interview you must have everything ready otherwise you will have to go again.
