I'm 40. Between 2021 and 2022 I got pregnant naturally but unfortunately ended up with miscarriages, due to blighted ovum and chemical pregnancy respectively. Therefore I started my first ivf in Nov2022 -
Eggs retrieved: 19
Day 3: 9
Day 5: 2 (4BB and 4AC)
In Dec2022 I transferred the 4BB, implanted but found out to be slow growing at week 7. So need to end the pregnancy with drugs.
In Feb2023, I started my second ivf:-
Eggs retrieved: 21
Day 3: 9
Day 5: 3 (4BC)
This time PGT-A was performed on the 4 blastocysts (all 3 from the second batch plus the remaining 1 from the first batch). I was told that 1 from this batch was unable to complete the biopsy on day 5, so had to wait until day 6 to see if biopsy could be done. On day 6, biopsy was successfully performed on this 1 blastocyst, and the grading changed from 4BC to 6BB.
PGT-A result came back, this 6BB and another 4BC from the second batch are normal. I used to have very stable period of 28 days, but seems the pattern has changed after my second egg retrieval (extended to around 33-35 days).
I have just transferred this 6BB but it was not successful, without implantation. So i would like to ask:-
1. Do you think for this time, the failure is likely due to embryo quality or my uterus condition? As last time implantation was successful, and this time the blastocyst is PGT-A passed.
2. Do you recommend any further checking for me? Or do you think I should start another ivf treatment?

1. Could be both. 6BB blastocysts have a slightly lower survival rate after freeze/thaw. And if you are using natural cycle FET, it is also possible for your implantation window to be slightly shifted.
2. You may want to do a ERA (endometrial receptivity assay) to see when to transfer your embryo. I have seen a good proportion of patients >40 years of age benefits from this test.
