Hello magaritams,

Other than the miscarriage 2 yrs ago, have you ever got pregnant before (with this same husband), any child?

If I were you, I would really give some serious consideration to 3 things:
1. How much do I want a child? Will it be ok if I remain childless?
2. How much time and $$ I should put into fertility treatment?
3. Will I be able to cope with the treatment, physically & emotionally?

Based on the sperm report, the result is somewhat borderline; together with the age-related decline in your egg reserve, I would proceed with more aggressive treatment should this iui fail. But there are couples out there whose financial situation is tight, and also due to religious reason they may not want ivf. If eventually you really want ivf, it's almost for certain that icsi should be used.

I still hope the coming iui will work for you. And of course, you are always welcome here & I'll try to help. Best wishes,

I can see some yellow jelly to release yesterday
and will see the new doctor in 29 apr
All I can do now is waiting and don't think too

Hi cutecutetown, thanks for starting this thread. I did one IVF but failed, have the following questions.

1. I am 31, going to be 32 soon. I'm really anxious of having a bb. The reason I did IVF is because of my bilateral blocked fallopian tubes. Other than this, everything else is fine in both my husband and my side (healthy sperms, good ovary / uterus, low FSH, decent uterus lining). The blockage does not cause any inflammation or watery. Doctor said that in fact I can consider doing a surgery to unblock the tubes, but there is no guarantee the tubes will be fully functional after the surger and the time i need to take to get pregnant. Since I am eager to have bb at a relatively younger age, i started the IVF. Which path does your patient encountering same situation usually take? IVF or surgery?

2. My last IVF was smooth. No overstimulation. Retrieved 5 large eggs and 2 smaller ones (not a lot because of less strong medication taken). All 5 eggs fertilised at Grade A and Grade B+. I put back three embroyos on day 2. My uterus lining was fine at 10.9mm. But none implanted! I don't know why!!!

3. Then, I did a FET the following month. However, both of my eggs arrested with 2 cells from 4 cells after thawing overnight. Does it mean that my eggs have stopped growing?

4. I can't believe the eggs deteriorated to poor quality from good quality after thawin. Was it related to the lab's technique is not good enough?

5. I am planning to do another cycle of IVF. In view of my overall sitaution described above, how can I ensure bingo this time? I don't want to waste more money and energy. I am doing in a private clinic by the way.

Cutecutetown, one more question. Is it better to use a day-3 or day-5 embroyos than a day-2? I think the highest success rate is using 5-day isn't it?

Then between day-3 and day-2 embroyos, would day-3 gives a better success rate?

Just married couples yrs ago. No child. But, I really want to have a baby now. I will listen to you. I already make appointment at PWH in Nov and try to have icsi asap. Nov, That 's a long line up. Don't you know any shorter private line? Thank you for helping me to make a right choice. Sometime people need some push.

1. I work in the lab and therefore all my patients take IVF. Based on what you described, a surgery should give you a good chance too; however, consult your doctor. I don't have the right expertise to advise which path you should choose.

2. It's a matter of chance. Although your subfertility background seems very straight forward, nobody is guaranteed a 100% take-home baby. Even natural conception is also a chance event.

3. Don't know if they had stopped growing; but they did not survive the freeze-thaw process very well.

4. Then you have to start believing, I am afraid. Appearance is not everything. Although I can't rule out the technical effect, I have seen countless cases in which the frozen-thawed embryo competency echoed the "quality" of the fresh embryos transferred. In other words, when a patient has good embryos for fresh ET and freezing, after she's given birth and returns for her frozen embryos, the thawed embryos are mostly of good to reasonable quality. On the other hand, for those who failed to get pregnant from fresh ET, the frozen embryos could still be good; in this case, just a matter of chance, the "gold" is simply hidden within embryos of the same ivf cycle. When frozen embryos did not survive well, this just tells me that the embryos of that same ivf cycle are not as good as they look.

5. Yes, ivf really costs money and energy; that's why you may consider repairing your fallopian tubes by surgery. But seriously, there is no way to ensure bingo. However, if you only have blocked tubes, whichever path you choose, you still have a very good chance. Sometimes you need a little more patience.

6. Regarding day-2 vs day-3 vs day-5. The longer I can observe the embryos in the lab, the easier for me to choose the best embryo(s) for ET. Note that the meaning of "best" refers to both the pace of embryo growth and morphological grade. On one hand, better selection can be made by growing longer in the lab; on the other hand, there may be a slight chance that no embryo will make it to day-5. Not all patients can accept such cruel fact that the whole cycle-worth of embryos are all incompetent. While this is very hard to accept and I also hate to see it myself, the reality is that it actually fast-tracks in time and allows the patient to quickly start over without wasting time in dreaming their bingo from any transferred embryo(s).

You will still have your iui next month, won't you?

I'm sorry that I need to stay neutral and can't really tell you which ivf centre to go to. We, as embryologists, more or less know who-is-who across centres and our laboratory services are very similar. In choosing which centre to go to, it is really up to your personal level of comfort, and perhaps a little of cost consideration, with the clinic's doctors and nurses.

In between your iui in May and ivf in Nov, you are not discouraged from trying to conceive naturally. Nothing is absolute in this world, unless you stop believing.

1. My embroyos not surviving the thawing process is indicating my fresh embroyos was not of great quality. Can doctor prescibe me some better stiumulation injections to enhance my follicles' quality?

2. I am starting my 2nd IVF cycle and aim for successful. Is there any way i can improve from previous cycles?

cutecutetown, forgot to ask you one important question.

3. when the embroyos does not survive to 5 days in lab situation, does it really mean they won't grow in my uterus. doctor's argument is the lab situation is not as good as our uterus environment. but i also doubt this saying as i see from some articles that if the embroyos cannot develop to 3 or 5 days in the lab also mean they won't grow in my uterus.

in this case, i will ask doctor to take the chance to develop the embroyos for a few more days rather than giving hope to myself.

1. It is a matter of looking back in time; there is really no way to predict before it happens. With the stimulation experience gained from your 1st ivf, as well as many other parameters such as hormonal levels and follicular growth, your doctor may fine-tune a few things in this coming cycle, which then should give better (or at least similar) outcomes.

2. Given the mild stimulation and the results you described previously, both your doctor and you had done the best you could possibly do. It's probably a bit of luck you need. Sometimes it might just be the cohort of eggs being recruited in that particular month that was not of very good quality.

3. This is forever a 2-sided situation where the debate can go both ways; your doctor's argument is many people's argument, and the articles you read about is the other side. There is nothing wrong to be conservative, and to be fair, I will see the lab's general success with day-2 or day-3 embryos before considering the extension to day-5 culture.

While I don't know how good your doctor's lab can grow embryos, I can tell you where my lab is at. In my lab and for most patients, we almost always see 4-cell on day-2. That's why it is always difficult for me to choose the right embryos to give you the best immediate bingo chance. My lab does routine day-3 ET; again, in patients with >5-6 embryos, we often see the majority at 8-cell or above. This is the time I want to do day-5, but sometimes the patient doesn't want it, or the doctor worries about the embryos not continuing to grow. I won't go into the details how we arrive at a mutual solution since there are other factors to consider.

In your case, I suggest that you do not need to be so decided yet. Simply look at how many fertilized eggs you will have; if you only have 2 embryos, you will have them transferred anyway, so which day almost won't matter. If you have similar number as this ivf, i.e. 5 embryos, it is worth to see how they look like on day-3. Then it will be up to you, your doctor, and perhaps someone from the lab to help you decide.

Happy? When the whole process is more transparent, it at least helps to reduce the level of anxiety.

Dear Cutecutetown, your illustration is very clear.
In fact, after I have gone through IVF and reading so much about IVF, I got the theory of the whole process. I want to be an infertility doctor or embroyonist too. The concept is not too difficult to grasp. Can I become an embroyonist at this stage? other than my biology is not great.

You are welcome to apply when vacancy will be available in the field. There will be lots of on-the-job technical training as well. Warning though - by the time you have a child to take care of, it may be a little difficult to cope with the almost-always working schedule on weekends and public holidays.

My embryo is released today but I am not sure that it is out my body completely or not since

I can't see my new doctor until next Friday (29 Apr)

Actually, I feel painful this morning but I can overcome it and the painful is disappeared now

Would you like to advise me :

1) Am I need to visit another doctor first to see my situation if no painful make me feel uncomfortable now?
or
I can wait next Friday to see the new doctor

2) How long will the bleeding last for and what can I do and don't in this period ?

3) What can I eat to recover myself faster after the embryo is released completely ?

4) Can I take Folic Acid and Vitamin now ? Which Vitamin is better for me to make easy pregnancy again ?
My IVF doctor give me some Vitamin E last time but some people said that Vitamin D is much better

I took eyes on Day 1 0930 while on Day 4 1500 ET, at the ET moment, there were 6 eggs fertilized from 8-cell to 14-cell. Doctor tranferred 8-cell and 10-cell. Which day will the embryo implant?Which day do u think I can check bingo? How about day 8? Thanks.

Dear lungb,

1) If no pain, you may wait till next Friday; however, if pain persists, go to emergency during this holiday weekend or at least visit your family doctor or a general practice doctor nearby to get medical advice.

2) This varies in different people. Do keep a healthy diet, eat some lean beef to help replenish your blood (just my opinion) and some dairy products (e.g. milk, eggs, yogurt) which boost up your proteins. Don't stress yourself and have good rest.

3) There is no faster way to replenish the lost blood. Health-wise, as advised above, eat healthy and do some moderate exercises if you can manage.

4) Folic acid and multi-vitamins are fine. Take the amount recommended on the bottles; taking too much is not going to help because the extra is often excreted or caused extra burden on your liver. Folic acids and vitamins are good for preparing for and during pregnancy, but no supplement claims to make pregnancy easier or faster.

1) I answered your same implantation question at least twice, my dear.

"Implantation time varies a little among women, but the general implantation window is usually from day5 to day8. In other words, it does not matter too much what stage or day you have your ET; your endometrium should also be in synchrony (at similar pace to suit the embryo stage). Simply count the embryo day until day 5 to day 8, this time frame is about right. Some references quote the implantation window goes up to day 12, although I hardly see any that implant so late."

So, once again, based on the way you counted (egg collection=day 1, ET day=day 4), implantation day should be some time around day 6-9.

2) Given the above, day 8 is absolutely not a good day to check bingo. Bingo test is a test on hcg level; so, if you are given hcg injections as post-ET support, an early bingo check will always give you a positive regardless of implantation. And in the case where implantation occurs, the little embryo produces hcg, but it won't produce so much so soon to be detectable; so again, doing early bingo check is a waste of your money in buying the test kit.

I assume your doctor must have told you a day to check bingo, just follow his/her advice. Checking too early is not good because the result is NOT reliable. What medication(s) are given to you from now till bingo check?

i'm using Endometrin twice per day. Doctor asked me to back to clinic check bingo on day 15. But usually ppl said that can check bingo around day 10-12, but anyway, if you advice to follow doctor's instruction, that I follow la...just 心急...hahaa...

Dear Cutecutetown, hope you have had a lovely holiday. Over the past few days, I have a few more ideas about IVF and would like to seek your opinion.

1. Does using ICSI has higher implantation rate than traditional IVF? my hubby's side is fine. Last time, we use traditional IVF and all got fertilised. But after defreeze, the embroyo arrested from grade 1 to grade 3. Hence, I think the embroyos may not have as high quality as superficial look.

I searched on internet saying ICSI gives better quality than traditional IVF as the embroyonist pick the strongest sperm.
Also, when the sperm is pierced into the egg, it created a small hole resulting in assisted hatching.

Are these two concepts sound?

2. does assisted hatching on day 5 result in higher mis-carraige? that's what my doctor said. assisted hatching results in higher miscarraige. I never heard of this on the internet?

3. finally, regrading transfer a day 3 or day 5 embroyos. i have an idea. i would transfer day-3 embroyos first for safety reason. then, if i have a few embroyos left, I would ask the doctor to continue cultivate it till day 5 and freeze the blast (if there is any embroyos left up to day 5). in this way, i have both day-3 and day-5 embroyos. hah.

i thought so hard over the holiday to think of the above ways. does it make sense?

or i transfer once at day-3, and transfer another day-5 in two days time if there is day-5 embroyos. which method sounds better?
