Dear Brittany,

Are you diagnosed by your doctor to have PCOS? Usually PCOS patients produced many many eggs. It can be up to high 20 to over 30 eggs. However, the follicles may be empty or the egg quality is very poor.

Age quality is usually related to a woman's age. How many eggs you had, how many get fertilized, and how many cells they divide into? What is the % of fragmentation?

If you are young and have less then 20 eggs, you may not have PCOS. It is normal. I had 11 eggs during my last cycle.

i am now 31.5 years old. i have pcos, which was confirmed by many doctors....however, after i did 腹腔鏡operation by stephine chxw (very money-oriented doctor)at my age of 28, it affected my eggs quatity......thats why everytimes i did IVF, i could only retrieve only around 10 eggs, and only 6 to 8 eggs can be fertilized.....and my eggs' quality are getting worse...

1 doctor said i have pcos. 1 said i'm suspicious of pcos
and 2 said i'm not. i had 4 eggs when having injection
of 400 iu for iui. is it normal? that's 6th iui, bingo but
miscarriage. (I bingo at 1st iui but also miscarriage).

if not many doctors sure that i'm pcos. can i take DHEA?
i have 6 iui and 2 miscarriage so many doctors said
embryo equality is the problem 'cos bb has stopped
growing at early stage (7-8wk).

i may do ivf 3 months later so i want to know if i can
take it and improve egg quality.

1) my period is usaully normal. only late 1 time in 2 years.
2) i'm normal size. height: 160cm; weight: 110 lbs.
3) i have oily skin but no pimples. i had many when i was
under 20.
4) i have some hair on face and belly but not serious.

sisters, if you have pcos, many doctors said you have it?

I do not know too much about the PCOS. I understand that it is a hormonal disorder. Because of hormonal imbalance, the ovaries are not function in a normal way. The following website describe the problem:

Therefore, you really CANNOT take the DHEA because PCOS women are already affected by high level of male hormone. The DHEA contains a male hormone precursor. If you take it, it may make your PCOS worse.

I think you should find a good reproductive doctor and hear his/her opinions. There are drugs that can help PCOS patients to get pregnant naturally without IVF.

Sometimes, PCOS patients use IVM instead of IVF. IVM does not use stimulation at all. When the follicles are about 10 mm, they will be taken out and incubated in the lab until maturity. Then fertilization takes place by ICSI.

From the statistics, PCOS women still have a good chance to get pregnant. Just need to find the right way and you still have time.

Usually there are two reasons for recurrent m/c.

1. chromosome problem
Before IVF, you can test both of yourself and your husband's genes to find out such a problem. One or both of you may have the problems. If it is confirmed, during IVF, you can select those embryos which do not have chromosome problems. Usually one cell is taken out from the embryo and a complete genetic analysis can be done. However, DHEA does not change the genetic makeup of the eggs.

Also, you can do a genetic analysis of the dead fetus when you have m/c. Then you sure will know whether that fetus is genetic normal or abnormal.

2. immune problem
It can be tested too. If it is confirmed, there are drugs to prevent the m/c. Usually heparin and aspirin are used. My doctor gave me heparin after ET in my last cycle, just in caseI had the problems.

Did you doctor give you any ideas why you have m/c?

If you do not find out what are the underlying causes of the recurrent m/c, IVF cannot help because the procedure can only have you to get pregnant but not change the genetic makeup.

100% agree with Frisbe, PLS DO NOT TAKE DHEA IF YOU HAVE PCOS....

If you are not sure, Pls check your Day3 FSH and your response to the drug, and whether your doctor told you that you are classified as poor responder to the stimulation drug in iui and ivf.....

Minka, have you checked your day 3 FSH ?

想問, 我下週驗fsh, 咁幾多度先叫高, 係咪高的話就可以食dhea? 我上個星期day23照到有七個卵泡, 想問有冇人知卵泡係會幾時消失?我一直以為排咗卵卵泡就會分解.....但我個e生又冇講咩..

Usually FSH is taken on Day 2 or Day 3. If it is higher than 10, it is considered to be high. However, only one month may not indicate the true condition of your ovaries. It is the highest one that tells you the condition. Like myself, I have one very high in Nov. last year, 14.7. However, all other months, I just got around 6. But the one, 14.7, indicates my ovary condition. That means the ovaries have difficulties in producing eggs.

On Day 23 and still have 7 follicles are not bad at all. Again, the number of follicles should be checked on Day 2 or 3 when the FSH is checked. When it goes up to Day 23, some of the original follicles may be absorbed back into the body. You may have more on Day 3 but by Day 23, it is only down to 7.

The most important thing is how many follicles will grow under stimulation. For example, if you have 10 follicles on Day 3. However, the stimulation can only help 3 follicles to grow to maturity. Then the results are not good.

At that time, you may consider taking the DHEA.

Hi 高齡產婦,

I just missed the wonderful report you had... how can I. So happy to hear that you already bingo. I am still working hard to reach this point.

Would you kindly advise which dr for 中醫同做針灸. I would like to see them too.

This month I am starting another IUI. My FSH level is decreased to 5.9, which was 8.3 last year. Dr. Tung also scanned 5 eggs which she said I should just go ahead and give it another try.

However, everyone please be aware that 高齡產婦 do
several things during that month before she bingo eg. 中醫, 針灸,運動.. beside taking DHEA. So her success may
not neccessarily due to the effect of DHEA plus DHEA will
have it best effect after taking for 4 months while she
only take for 3-4 wks, so I really wonder whether it is really the effect of DHEA at all.

I just want to point out that, if the condition is suitable, it may be good to try DHEA. However DHEA is not a magic drug and it do not work for everyone even if the condition is suitable.

Let me clarify about the DHEA effects. DHEA reaches its peak effect in 4 to 5 months means that its effect increases to the highest point in 4 to 5 months. However, based on the medical report and my personal consultation with the original researcher, Dr. Gleicher, the DHEA can have a positive effect (not peak yet) before 4 to 5 months. Dr. Gleicher saw some of his patients got pregnant during the first few months. Therefore he recommends couples to continue to have active sex because when the egg quality is improved, the woman can get pregnant easier.

Before I took the DHEA, I had only 1 egg in one cycle. My egg quality and quantity were improved after 5 weeks of taking it. After 10 weeks of taking the DHEA, I got 11 eggs.

So the DHEA will start having some positive effect after taking it.

OK, DHEA may start to have its effect right away. But in 高齡產婦 case, she did a lot of things like chinese
medcicine, accupunture, exercise, massage..etc. Also she has also bingo a few months ago without taking DHEA. So her success may not be due to the effect of DHEA at all.

Taking about DHEA is so effective and many people here
say that they improve their egg quality and quantity....etc.
But then did anyone here really bingo and have a life birth and proven it is the effect of DHEA. I didn't see it yet.

So I think it is better to have some really first hand
experience here to share instead of just report. Report is good for reference though.

I 100% agree with Judith07 that DHEA is not a magic medicine which can help whoever has the egg quality problem. Using DHEA in fertiltiy treatment is relatively new in HK. However, if you search "ivf" +"DHEA" over the web, you will find that it is very commonly use in the US now and there are a lot of successful stories. I guess there are more older women who are trying to conceive in the US than in HK. I have to admit that DHEA does not work for everyone, that's why HK dr will not recommand it as it is not 100% clinical proof effective. (There are still some doctors in HK will recommand it, my 2 ivf doctors recommanded me to try actully) Just like there are still a lot of fertilty doctors in HK do not really believe Chinese medicine.

I think whoever is thinking to try DHEA, pls understand your own situation before start taking it. Not everyone should take this supplement, especially who has PCOS . Pls have a look of the following information Frisbe post before:-

1. Who should take DHEA?
a) Women who are older than 40 or
b) younger women who produces less than 4 eggs under high dose of stimulation. They are consider poor responders.

2. How long should you take it?
You can continue to take it when you are doing ivf cycles. No need to stop during stimulation and after embryo transfer. Just continue to take it until you are confirmed pregnant.

3. Dosage of DHEA?
He recommends us not to take the over-the-counter DHEA (like the ones we got at Mannings) because we do not know how much can be absorbed in our bodies. His patients use micronized and pharmaceutical grade, 25 mg and 3 times per day. Total 75 mg per day. You can order it from the U.S. Many places carry it and I found the following website useful about this kind of DHEA.

4. How long will it take effect?
It usually takes 4 to 5 months for it to reach the peak effect. It is better to do ivf after 4 to 5 months taking it.

Yes, you are all right! I really did a lot of things before getting bingo, and one main thing is maybe I'm quite relax during that month as I'm just looking forward to the coming IVF in May.

So I recommend you try your best to do everything which you think it should be good and can help you, and make your body fit for pregnancy. Just do whatever you can!
