Dear biglemon,
You probably won't need to repeat the sperm analysis. I may not sound objective enough, but in my own lab, if I see these numbers from a sperm preparation for an IUI procedure, I simply get frustrated and pray for a miracle to happen. I understand there could be a lot of reasons why a couple chooses to do iui instead of icsi and I respect these reasons. But if you don't have any particular reason for delaying ivf/icsi, don't delay it.

Good on your husband and I want to give him a "Like". These measures will likely improve sperm "quality" not "quantity"; but most important of all, he can get more rest and a "healthier" lifestyle. Going for ivf/icsi is a joint decision by you and your husband; so, talk to each other a lot before deciding and support each other in whichever decision.

First of all, thank you for spending your time on reading all our questions.

I am a newbie in IVF. Actually, I just have two attempts of natural IUI, but did not succeed in both.
According to my doctor, my body is fine. The problem comes from my husband. Based on his sperm tests, they shown that his sperm quality and quantity is not meeting the standard.

Therefore, I hope you could help me take a look at his reports, and suggest should we go directly to IVF. I am 32 yrs old and he is 37 yrs old

Test 1:
Days of Abstinence: 4 days
Volume: 3.5 ml
Viscosity: Increase 1+ (actually, I don’t know what this means)
Progressive motility: 40%
Non progressive motility: 7%
Density : 41 (x 10^6ml)
Morphology 1%

Test 2:
Days of Abstinence: 2 days
Volume: 1.8 ml
Viscosity: Increase 1+
Progressive motility: 20%
Non progressive motility: 5%
Density : 14.6 (x 10^6ml)
Morphology 1%

Test 3:
Days of Abstinence: 0 days (doctor suggested us to do a 2nd insemination the next day)
Volume: 1.7 ml
Viscosity: Normal
Progressive motility: 24%
Non progressive motility: 6%
Density : 19 (x 10^6ml)
Morphology 1%

Also, I have the following questions:

What factors would determine how many eggs my body can “release”? is it my body reaction to the medicine? Would health condition and age matters? How can I increase chances of having more eggs prepared?

Does the doctor always pick the best quality embryo to place into the uterus, and then freeze up the rest there is “less optimal”?

What could be done to increase the chance of allowing the embryo to implant into the uterus?

When two embryo is placed into the uterus, how likely both of them will stay and turn into fetus and become twin? Is it age, health condition, that determine the likelihood?

How can my husband improve his sperm condition? Would sleeping early help?

I realized I have asked you a lot of questions, and I am looking forward to your help. Have a good weekend.

I would definitely tell the dr about my fibroid if bingo
For the ET this time, although everythings seems going smoothly, I don't want to have high hope since I únderstood many women has better change to bingo when having FET. Of coz, I wish i have the luck but I am also well prepared for the failure....I am not being pessimistic, just that don't want to have big disappointment when the judgement date comes!

I finally canceled my Hatha 1 bookings and booked Yin for today and tomorrow.....90% of postures are lying on the floor, very good indeed! In fact these few days I am feeling a bit fatigue, so very vigourous class proabably not my cup of tea.

Once again, thank you for your suggestions and advise all the way through, no matter it is a bingo this time or not, I really appreciated your support!
Will keep you posted of my result

If you want, you may still try 1-2 IUI's while considering whether you are prepared to go for IVF/ICSI.

The number of eggs each IVF cycle, or stimulated cycle, can release depends on your ovarian response to the medicine. Age matters, and then other factors. Health-wise, what is healthy anyway? Everybody is a little sick, we are constantly under stress and there are chemicals and pollutants around us. If you manage to keep a somewhat healthy lifestyle and stay away from smoking & drinking, you are likely fine.

Now, a lot of jm's want many many eggs per IVF cycle to increase the "success" chance. To be more meaningful, it's way better to have egg quality than quantity. Having 2 good eggs is better than having 20 useless eggs (this might be an extreme example but I hope you get my point). You probably can't increase the number of eggs by doing/eating something extra; and there is no last-minute remedy. In the hope of having some good quality eggs, try to lead a healthy lifestyle is both good for fertility and overall well-being. I often tell jm's to do moderate exercises, which will lead to better blood flow to oxygenate the ovaries (where your eggs are nurtured) and the uterus (where your potential fetus will implant in the first place). If you are not the exercise type, try to take a walk after dinner or go to the gym for a quick 30-minute session on a treadmill doing brisk walk will be beneficial.

I can't say for all doctors, but most will pick the best-looking embryo(s) to place into the uterus, then freeze the remaining. Note that you may have, for example, 5 good-looking and 2 average-looking embryos; then you don't get all 5 at once. You will have perhaps 2 first and then freeze the other 3 plus the 2 average embryos.

Possibility of multiple pregnancy always exists if you put more than 1 embryo back. Chance of multiple is higher if a woman is young, doing IVF first time, and/or has plenty of good-looking embryos. The percentage varies for different age groups; you may ask your doctor to quote you specific numbers.

For men, especially those living in HK, most of them are working (too) hard and sleeping late. Sleeping early may help, if it gives more quality resting time. Try to stay away from alcohol or cigarette smoking; watch eating habits may also help. Again, if these may help a man's sperm, do not expect a dramatic improvement. Usually sperm morphology doesn't change much.

Cutecutetown, many thanks for your reply.
I will meet with the doctor again to see if it is still a good option to go for another IUI, or go straight to IVF. She mentioned that IVF is more helpful in our case, so we end up with going directly to IVF. I just want to know the chance of success through IUI, given my husband sperm quality.

I had 3 OI IUI last year and got bingo on my third IUI but got m/c around 9 weeks. i am having my last IUI attempt this cycle after 4 months of resting and exercises. My current doctor said this is the last IUI try. and I should consider IVF afterwards. and my husband and I agreed with him.

My question is what should i look for when choosing an IVF doctor and his associated facility (many ppl go with name and by referral). what types of questions should be asked to interview the IVF specialists. my husband and I are normal and passed all tests but my age is the citical factor (I am 40). we live a very healthy lifestyle, eating organic, 3 times exercise weekly, no smoking or drinking except sleep quality is lacking. i have been eating holistic prenatal vitamins as well. any suggestion for me. Thanks!

Dr said I will have the OHSS, since a lot of eggs in my left ovary. I have searched OHSS in the website, it is kind of dangerous and will causee to death.

I have injected the HGC yester night and will have the operation tmr morning to get the eggs. I want to ask what I can do to cope with ithe OHSS. Thanks!

I thought I wouldn't bother you again before my PT next week and just to let you know my result....but I have a problem that really need update you...

I think I am suffering from late onset OHSS...OMG........I am feeling so uncomfortable suddenly from yesterday morning, my stomach bloated, lost my appettite at all, and nausea
I couldn't sleep on my side but need to lay flat as my stomach was in pain in any other posture. I thought I have indigestion or heartburn, so I took a medicine that was prescribed by my doctor before....a little better after taking it, at least my stomach was less bloated when I was in bed! But this morning, the bloating seems getting worst and I just can't take any breakfast at all as I feel like having a bag of rice stuffed in my stomach!!!!
As I was quite hungry by lunchtime, so I ate a bowl of soup noodle (米線), and I was so regreted afterwards as my stomach bloated even bigger with the noodle inside.
So, I called QM this afternoon and told the nurse my problem, she said I probably suffered from late onset of OHSS and asked me to go back tomorrow morning to see the doctor.
May I ask if it is common to have late onset OHSS, and if this will affect my chance of bingo? I surfed a bit on the internet and found a lot of discussions about late on set OHSS, and some even said this indicates high chance of bingo...is it true?
My ET was on the 10th, and my PT appt at QM is scheduled on the 26th, will it be too early to do PT today or tomorrow?

Hello bkoon,
It is a very personal experience in choosing an ivf doctor. Even for the same doctor, some may find him/her very good while some others may find him/her not helpful. Some doctors may be famous but with a super long waiting line, while some may not be very famous but willing to spend more time in explaining info related to treatment. My advice is to follow what your own mind tells you after you see a doctor, don't base your decision on what I tell you or what other jm's tell you.

As you mentioned, the critical factor now is your age. No matter how healthy you have been living, we cannot rewind the biological clock. Your healthy lifestyle probably has helped, but egg reserve simply diminishes as a woman ages. So, proceed to IVF quick if this IUI fails; don't spend too much time thinking and waiting.

OHSS is dangerous and could be life-threatening in extreme cases. Nowadays doctors are very cautious so that they will keep you under monitoring especially if they anticipate you are at risk.

The egg retrieval will still proceed, then eggs get fertilized and all embryos will probably be frozen for your later use. You may receive some medication during and after egg retrieval to prevent OHSS, and your doctor will keep you in check. After the egg retrieval, your doctor will usually tell you what you may need to do. Some private doctors will ask you to eat more proteins if symptoms are mild, while some may need to admit you into the hospital simply for monitoring.

It looks like OHSS by your description, but as you will be going back to QM tomorrow, they will take proper care of you.

Sometimes OHSS just comes in late. It won't affect your chance of bingo, although bingo will often make OHSS worse (more bloating and fluid in your peritoneum). I am just not in the right position to tell you how good/bad your OHSS is or whether you have bingo or not because what I say now will only lead you into a lot of thoughts and therefore a sleepless night. Generally, you will do ok tonight; then you will be back in QM tomorrow with proper medical attention. As far as your own health is concerned, you are in good hands. And for your bingo, the hospital will hopefully check it for you.

I went to see the dr at QM this morning, and confirmed I have OHSS definitely but not serious although I feel the symptoms severe to me. She did U/S on my tummy and found my right ovary is more swollen than my LHS and has fluid inside, but mentioned nothing if there is "anything else" inside.

She told me be patient as there is nothing we can do with OHSS except to wait for the symptoms diminish by itself over time; and asked me to go for another check up this Friday before my PT on next Tue.

I guess maybe I have taken 3 egg white and drank a bottle of Pocari + a few glasses of

Electrolyte water last night, so the bloating seems not so severe today compared to yesterday. But the dr told me I am still quite dehydrated when she checked my tongue.

She also reminded me that if I really got pregnant, these symptoms may persist for another few weeks time........

Good to know that this late onset OHSS will not affect much on the bingo.

May I check if there is any case you know a woman got pregnant but only suffered a short period of late onset OHSS, says about a week or so, and still have a safe and sucussful pregnancy? Or, it would probably be a few weeks of suffereing with not much variations?

P.S. I couldn't sleep well whole night long as no postion is perfectly comfy, so I already have a lot of thoughs even you didn't say anything......hope thing is getting better gradually.

Will have to see how these few days go. With the hcg injection (安胎針), your bleeding should cease.


It is very hard to tell whether or not the embryo/fetus will hang on. For early miscarriage, it is often the embryo that is the problem, not the woman. I read your previous posts that your job gives you high stress. I don't think simple walking can cause miscarriage, especially you can take rest whenever you feel like it. But if this becomes your concern, don't do it but try to slow down and even lie down whenever possible. You mentioned 煲劇 before, which can be good if you don't stay up all night for it!


There is not much you can do but take rest whenever possible. Rest means slowing down your pace, trying ways to relieve your stress, bed rest, or a combination of all. Life can be stressful if you work; at work, when I feel stressed, I often tell myself to slow down, take a few deep breaths, or take a moment to just sit and do nothing.

You can't see "anything else" with this U/S because even if there is "something", it is way too small to be visible on U/S.

At least you are not hospitalized, so, your OHSS condition is not too severe. Well, I know you still feel very bloated; but in its severe form, a woman will be in the ICU, not allowed to go home!

If a woman got pregnant, the OHSS symptoms will not go away in a week. There will be a few weeks to "suffer" the same symptoms, but then the woman's happiness takes over the sufferings of the symptoms. When under close monitoring, the pregnancy will not be affected. There have been many women who had OHSS in their early pregnancies and yet the babies were born healthy. So the woman's health is to worry, not the embryo's in OHSS cases.

I hope you can sleep a little better, stay hydrated.

So I\'ve had 3 ivf already, one turned out to be chemical pregnancy. no reason why they failed. If I keep doing it, is the chance very slim? So I plan to do one last ivf with PGD to find out if its paternal or maternal issue. Based on result, can move to donor sperm or egg. What do u think of my approach?

I had my FET (4 cell) done on Mar12. And I have been having on and off brown spotting since Mar19 evening. Do u think I've failed it already? I did call the nurse today but she said there is nothing we can do now. Is it true that nothing can help? Also, I had a Pregnyl injection on Mar18. And I wonder how long it will last? As I really want to do a urine test at home tonight, but afraid that I might get a false positive due to the injection.

Dear jlin123,
With PGD, it may help to identify a normal embryo before putting it back into your womb. Where will you be doing PGD?

If I get it correctly, the PGD you referred here is an aneuploidy screening, which is to check an embryo if it contains the correct number of chromosomes. In this case, you can't narrow it down to paternal or maternal issue. Do you or your husband have a known family history of inherited genetic disease?
