Yes, 我自己打破卵針。一開始時，兩邊各2個。有case 再造會成功嗎？我之前一兩個月有重感冒，身體不太好，加上我今次都幾stressed, 唔知會唔會因為咁，身體唔ready... 我想我地有心理準備放棄了

HI Cutecutetown,
Just to let you know that my PT at QM confirmed positive on 26/3, and my OHSS still exists and it has been 20days since it started. The bloating fluctuates day by day, one day I felt better which I thought the fluid was started to go away.....but it came back again the next day....so unpredictible

As my first U/S at QM is scheduled on the 11th, but i was worried about my OHSS so I went to see a private dr at Sanatorium on the 2nd.
The U/S showed my abdormen is filled with fluid, my ovaries are swollen to 7cm

, lucky that no fluid in my chest/lung, so the dr told me my OHSS is not very serious, and i really have to wait for the OHSS to go away slowly by itself. The u/s only showed a sac, and the size of the foetus is 1.11x0.91cm. The dr didn't say much about the details except she said a 胎芽 can be seen.
To your experience, is the foetus of right size at 5 weeks? Do you know normally when a blink blink heartbeat can be seen?
The dr didn't ask me to do any blood test for my hcg level, is this normal practice not to test hcg if foetus can already be seen from u/s ?

Good to hear that you are doing fine.

Even in cases without OHSS, the ovaries are somewhat enlarged due to the drug stimulation. The swelling will subside, although very slowly.

Hm.... I believe the fetus size is about right. It will need approx. one more week before the heartbeat can be visualized on the ultrasound.

Can't tell if this is normal practice not testing hcg; however, this Sanatorium visit would probably be your only one and a stand-alone hcg wouldn't tell much in your case. The doc probably knew you would return to QM for a scheduled appointment anyway. No worry, he/she saved you some expensive blood test fees at Sanatorium.

你好呀我睇左你d post 好耐，好想請教下你少少問題呀

我28歲，16歲因卵巢生水long 切左一邊卵巢，因先生精子好差，所以威院醫生好快建議我做ivf+icsi。係12月初抽完卵，抽左31 粒，但得4粒受精， 2粒8 cells, 1粒7 cells, 1粒6 cells, 威院話佢地無分grading. 來緊放雪胎啦，我好擔心想請教以下問題：

1) 我得番一邊卵巢，醫生話功能ok, 但少左一邊會影響著成功機會嗎？
2) 點解抽31 粒，做icsi 受精率都係得5粒咁低？
3) 一次過抽31粒，會否因此卵子質素降低，就算bingo 都會較易小產？
4) 8 cells 機是否比較大呀？

唔好意思，問咁多問題，感激萬分！

1）卵巢與著床成功機會沒有直接關係
2）得一邊卵巢也有31粒蛋，得4-5粒受精，你知有多少蛋是成熟的嗎?只有成熟的卵子才可做icsi。
受精率低可能與精子差有關；但若你所說，只有4粒受精、而4粒都能達6-8 cells是不錯的。
3）我估計你31粒蛋可能有頗多是不成熟的，通常這麼多蛋的個案卵子質素會較差。卵子質素差通常減低bingo機會，但若bingo後是否會較易小產也不一定。
4）8cell 固然好，但7cell也不要「睇小」; 6cell 在我的實驗室要視乎碎片程度多少，靚的6cell(沒有／很少碎片的）也有一半機會到第五天生長達標，給我囊胚(blastocyst)!

Hello sara_chu,
沒有特別文獻或research說bed rest對著床有幫助。我亦見bed rest 是見仁見志，隨心吧，心理有時影響生理的！
黄體足夠與否要驗血才知，一般月經周期準都不會／不須懷疑自己黃體不足，除非你的醫生有這麼說。

我印象中姑娘同我講31 粒入面有十幾粒拎去做icsi, 得4 粒受精。我真係好擔心會因為卵子質素唔好，曬左來緊幾次放胎機會。醫生之前話我年紀唔大，侮次放一個胎ja, 再加上卵子質素唔好，好驚bingo 唔倒>

dear cutecutetown,

aiya! so after 4 days, i repeat the hcg test. it only went up from 61 to 66! but i have no bleeding or pain. doctor ask to repeat the test in 3 days. she said since i transfer 3 embryos there, there could be something going on that lead to hcg not rising as expected. what's really going on??? what are all the possible outcomes? help!

the weird thing is the no bleeding part. at least it was bleeding in my past 2 chemical pregnancies.

Dear jlin123,
This is frustrating

There could be 3 possibilities I can think of, although the answer will reveal itself after you test in 3 days. If this will be a miscarriage, you should start the bleeding soon. However, if in 3 days time your hcg still goes up by just a little bit, there may be a small chance of an ectopic pregnancy. The 3rd possibility is the least likely, an erroneous hcg result of 66.

Sorry having to prepare you for undesirable outcomes. In the worst case, you still have a frozen normal embryo.

What if the number doubles again in 3 days? Have u seen cases when hcg first doubles, then up only a little and jump as normal? Or am i only dreaming for miracle...

But I have no pain at all, not even one side. Or ectopic can still happen without pain and bleeding?

Hi cutecutetown ,

I have started my first ivf yesterday ! Hope everything will be alright and end up with victory 😊

I want to ask is there any suggestion about timing of sex before my husband " hand in " sperm "? How many days of sex abstinence before that big day is needed ? Like some people said maybe have sex few days before that big day to generate more healthy sperm .

Dear biglemon,
I don't think you and your husband should be having intercourse while you are on ivf stimulation. I believe you mean the time lag or period of abstinence, which is the time to refrain from an ejaculation. Usually, this period can range from 3-7 days.

Actually people said don't leave sperm for too long, so just wonder if he need to 放丁few days before I 抽卵，so to provide better specimen 😊

hcg result now down to 11. so you are right. once it doesn't go up, it'll just drop. but it's still a mystery to me why there's no bleeding or cramping when miscarriage occurs? unless the endometrium really stop all the bleeding.

anyway time to move on! so i have a few questions here for my next plan:
1) so i want to do FET next (the 4BB from last lVF). do frozen embryos need time to rest before thaw or i can just do it now?
2) also for FET, should i do natural transfer or with IUI together? which has higher success rate?
3) icsi vs natural fertilization: in my case, i seem to have good fertilization rate due to ICSI, but the implantation/growth rate is not as good. does it make sense if i ask dr to do half ICSI and half without? even though the fertilization rate may be much lower, at least the strongest sperm will make it through, therefore reducing # of abnormal embryos!

I am glad that you can rationalize so soon. I am still a little sad as I was once being very hopeful too. Anyway, the bleeding should return very soon, sometimes it may take >1 week or even 2 wks.

1) You may just do it in this cycle. However, I would rather give your body a 1-month rest. Let this bleeding be over, then do the FET in the next cycle (your next bleed in May should be your next cycle). Of course, see what your doctor say as well; he/she may have other reasons to FET later.

2) I have never worked with doctors who do FET and IUI at the same time. I am never a big fan of IUI, unless the sperm is super good. Even with super good sperm, FET (with a blastocyst) still gives a better chance than IUI.

3) ICSI delivers a sperm directly into an egg, so, if the sperm is not extremely poor, fertilization is almost a certainty. However, you had an IUI pregnancy before and it is logical to think that IVF may work. The only unknown is that if you do PGS, the center may need to do ICSI simply because sperm cells may cause misdiagnosis (although the chance is very low depending on the PGS method used).

The technique ICSI itself does not cause more abnormal embryos; the seemingly higher abnormality in ICSI is probably due to poor sperm.
