PCCW termination form

Anybody has this form for home telephone? They said they will only mail to me and need to wait for one month for termination.

Also, any comment on Hutchison service? Is it stable? What about their internet service? I intend to switch to Hutchison. Does it worth?

我用左和記大約5,6年, so far ok ka.
