in fact, all agent have the sample of termination letter and letter to Immigration Dept, you can ask them. For the date of air ticket, as you terminate her and not becasue of not renewing contract, thus your date should be 5 Jan or 6 Jan the latest. Ask her to leave HK as soon as possible, it's more good to you.
I have the same situation like you. After consulting my agency, they said I can give the maid an air-ticket which departure on the same day I terminate her, but the ticket must be open, which means that if the maid does not want to leave HK on that day, she can change the departure day by herself. Because by law, she has the right to stay in HK for 14 days after you terminate her.
However, in my case, I have to buy an ticket which departure day is 6 days after I terminate her. The reason is because the policy of the air-line stated that if you want to change the departure day, you have to notice them 5 days in advance. That means if I want to terminate my maid on 5 Jan and give her the air-ticket on the same day, then I've to buy an ticket which is departure on 10 Jan or later, otherwise, the maid will not be able to change the departure day.
Well, it is only my case, you may consult your travel agency first, different air-line may have different policy.
