I think I am quite lucky as I just employed a maid with all your requirements in late January. I paid $1,5xx altogether, including working visa, consulate fee, medical check-up (optional) and delivery charges.
Actually, you still have some time to select a good one as my maid came after only 4-5 weeks from date of contract signing. Good luck!
